How to Register

If you live in the practice catchment area you are eligible to join our list.

To register permanently with a general medical practice (GP practice) you’ll need to complete a registration form.

If you are newly registering and need medication on a regular basis, you will need to make a GP telephone appointment in advance, before it can be issued.

Acceptance on to the list is as at the discretion of the Doctor and we may ask you for proof of identity. (Please refer to our registration policy below and why we ask for this). Please note that your medical records may take several weeks to arrive at the Practice.

Patients register with the Practice and not with a named doctor. You may choose to receive services from the doctor of your choice unless he or she does not routinely perform the service in question within the Practice.

Medical treatment is available from the date of registration. Please contact reception for further information.

The practice has a non-discrimination policy for accepting new patients. There are a number of reasons why you may not be able to register with your chosen GP, for example you may live too far away. If this is the case simply choose another GP in your local area.

It is is important to note that you can only permanently register with a GP practice if you’re staying in the area for more than 3 months.

Please note that if you were previously registered with a Doctor outside the UK then the Practice will not have access to your medical notes. All patients who register from abroad should contact their previous Doctor to obtain copies of their notes.

Once you have completed the form please upload the form to the practice.
